    You can use this transaction if an existing lease between your
    company/corporation and another entity or owner/operator has been broken
    or terminated. The vehicle owner's record will immediately be updated to
    reflect the date of termination and the reporting Motor Carrier's name.

    NOTE: This statement can not reflect a lease termination date of more than
    15 days prior to today's date.



    In order to submit the Notice of Lease Termination, you will need the following information:
    • Vehicle Identification Number
    • Model Year
    • Name of Motor Carrier (Lessee)
    • Date Lease Was Terminated
