How do I replace a driver's license or identification card that has been lost, stolen, or mutilated?

​​A. The requirements to replace a lost, stolen or mutilated driver's license are:

Adults
  • one primary document and one secondary document OR
  • three secondary documents
  • Proof of insurance is required for issuance of the duplicate driver's license if you own any vehicles.
  • Proof of Residency
  • Fee: $13.00 (a local fee may apply which will not exceed $4) for a 4 year license and $17.00 for a 6 year license (a local fee may apply which will not exceed $6).

Acceptable methods of payment are as follows: 

Credit cards, debit cards, money orders. A transaction fee will be assessed for payments made with a credit/debit card as indicated below:
Credit/Debit - $1.25 plus 1% of transaction total​​​

Minors
  • Certified copy of the birth certificate
  • Custodial parent and their identification
  • Fee: $13.00 (a local fee may apply which will not exceed $4) for a 4 year license and $17.00 for a 6 year license (a local fee may apply which will not exceed $6).

A complete list of acceptable identification documents is in Section 1, Policy 6.00. In case of joint custody of a minor, only the domiciliary parent may sign. We do not process this transaction online. The charges for a duplicate driver's license will be waived if the license has been stolen and a police report is submitted indicating such.
